Comprehensive Health Assessment Test is a new service offered by Theresa Dale  Theresa Dale is a Naturopathic Practitioner and does not diagnose or treat disease. She can however accurately assess your nutritional needs and suggest lifestyle changes.

This extensive test and questionaire will enable Dr. Dale to recommend an Individualized Nutritional Program including necessary nutrients. Dr. Dale does not sell any nutritional supplements. You will be provided the information necessary to purchase these supplements and foods from health food stores, manufacturers and distributors. There are over 600 very detailed questions you will need to answer. In addition, you will be given an appropriate individual diet. We will email your personalized diet or we can FAX it to you within 7-10 days after you complete the test.

This individualized program includes a 5 minute phone consultation with Dr. Dale after you receive the results of your test.

The cost for this confidential assessment of your health is only $150.00

Ask your doctor or lab to perform a SMAC-24 or CHEM-24 blood panel, a Complete Blood Count and a Complete Urine Analysis, which you will then mail with your completed Comprehensive Health Assessment Test. Follow the mailing instructions that comes with the assessment test. When Dr. Dale receives it she will create your program based on your lab test using "optimum reference ranges". Your program will include suggestions for individualized nutrition, diet, supplements, dental healing, environmental, exercise and lifestyle changes.
