How do I request appointment?
Look for the red icon on the Members dashboard. Click on ”SET AN APPOINTMENT WITH DR. DALE”.
Does Dr. Dale see patients in person?
Yes. Her office is located in Ventura, CA. First appointments made in person will be 1.5 hours.
I do not live in the area. Can I still make an appointment?
Yes. You can request an appointment by telephone or by Skype video chat. First appointments are always one hour.
How much are the appointments?
In Office Consultation
First Appointment (One hour and 30 minutes): $450.00
Additional Appointments (One hour): $395.00
Phone or Skype Consultation
First Appointment (One hour): $395.00
Additional Appointments on a 30-minute basis: $197.50
Are products included the appointment price?
No. Products are purchased separately.
What is her appointment schedule availability?
Dr. Dale is available for patient appointments on Tuesday, Wednesday and Thursday: 9am to 11:30am PST and again at 2pm to 3:30pm PST. Please call her office at 800-219-1261 for current schedule availability.
Can I just call the office directly and schedule the appointment?
No. You must request the appointment through the members area as directed above. Dr. Dale’s office will contact you once the request is received.
How do I know that my appointment request was received?
Dr. Dale’s office will contact you to confirm appointment date and time.
How many appointments will I need to make?
This will vary from person to person. Typically, after the first appointment there is a request to follow up in one month. If you are sending in lab reports or blood tests to be analyzed, this can take done in shorter increments of 15 or 30 minutes.
I need to change/cancel my appointment.
Please call Dr. Dale’s office at your earliest convenience to reschedule. 800-219-1261. Appointment changes require 24 hours notice. Missed appointments will stand as charged.
I have an extra time credit from my appointment. Can I use the credit towards my product order?
No. Time credits can only be applied to future appointments.
I sent in my blood tests results for review. When will Dr. Dale contact me?
Dr. Dale will review any lab work during a scheduled appointment. Please request an appointment if you send in any labs for review.
What does Dr. Dale need prior to my appointment?
A 6-page health history form will be sent to you via email. Please complete and return to the office via fax 805-830-1107 or email firstname.lastname@example.org.
It’s also helpful if you have a saliva test and/or blood test results to submit prior to your appointment. The Five Element saliva test can be found here.
Blood tests routinely recommended include the CWP and the Dale Panel found here. Please contact DirectLabs directly with questions regarding ordering blood tests.
Do I need to do a saliva test and/or blood work prior to my appointment?
No. This is optional. Only a health history form is required, but you may be asked to complete labs if necessary.

How long does it take to receive my saliva test results?
I don’t understand my results. How do I read them?
The therapeutic recommendations attachment correlates directly to the lab results attachment. There is a product glossary and dietary guidelines for your convenience. If you have any further questions, please make an appointment with Dr. Dale.
How long will have to order the recommended products?
This can vary from person to person. With an appointment, Dr. Dale can specify dosage and usage instructions.
How do I ask Dr. Dale a question?
Look for the question mark icon on the members dashboard. Click on “ASK A QUESTION”. Dr. Dale will respond to your question within 24 to 48 hours.
When does my membership expire?
Wellness Center memberships are automatically renewed every 90 days. There is no expiration date.
How do I cancel my membership?
Log into your Paypal account. Since your Paypal account has confidential information, you will need to cancel your own subscription.
